On receipt of this form Vonage will respond within 2 business days. Termination of your Vonage Service (either in full or in part if you have requested termination of some but not all of your lines, seats, extensions or other features) will occur on the date you have requested provided you have told us that you do not wish to listen to offers and have indicated that you have understood the important information at the bottom of this form below. As set out in section 3.2 of our Terms of Service, early termination charges including other outstanding charges may be charged when you terminate before your contract Minimum Term has expired. You will receive confirmation of your termination by email.
